Required consents and notices for treatment are available upon request when contacting J4Hope or assigned therapist. Below for your review is the Notice of Privacy Practices and No Surprise Act Notice. New clients must call to schedule their first appointment. Forms can be completed and applicable documents can be signed and uploaded to our client portal. For those using our client portal, you will be emailed a link with instructions for logging in as an active client and completing intake documents. All intake forms and documents must be completed and submitted before your first appointment. If you have any questions, feel free to email or call J4H.
Active Clients may access the Client Portal features. New Clients can only see available appointments, but must call to schedule first appointment.
