Below are required consents and notices for your review that new and active clients are provided before and during the course of treatment. New clients must call to schedule their first appointment. Forms can be completed and applicable documents can be uploaded to our client portal. For those using our client portal, you will be emailed a link with instructions for logging in as an active client and completing intake documents (no need to download forms below).
